ALGEMENE VOORWAARDEN BEHANDELING
- De therapeut is gehouden te handelen conform de wettelijke regelgeving. De rechten en plichten
van cliënt en
hulpverlener zijn onder meer vastgelegd in de Wet op de Geneeskundige
Behandelingsovereenkomst (WGBO) en de
Wet kwaliteit, klachten en geschillen zorg (Wkkgz).
- De therapeut houdt een cliëntendossier bij. De cliënt heeft het recht tot inzage in het eigen
dossier. Het dossier wordt
conform de wettelijke bewaartermijn in totaal 20 jaar bewaard.
- De therapeut verplicht zich informatie betreffende de behandeling te verstrekken aan de cliënt in
alle fasen van het
behandeltraject. De cliënt verplicht zich middels het intakegesprek relevante
informatie aan de therapeut te
verstrekken.
- De therapeut betracht geheimhouding ten aanzien van door de cliënt verstrekte gegevens.
Echter met uitzondering van
de gegevens waaromtrent de cliënt heeft verklaard dat hij/zij geen
bezwaar maakt tegen het gebruik voor statistische
doeleinden.
- De therapeut mag zonder toestemming van de cliënt geen bepaalde (be-)handelingen verrichten.
- De therapeut verplicht zich de cliënt correct door te verwijzen naar een collega-therapeut of een
arts, indien haar
behandeling niet geëigend en/of toereikend is.
- Beëindiging van de behandeling kan te allen tijde met wederzijds goedvinden geschieden.
Indien de hulpvrager voortzetting van de overeenkomst niet langer op prijs stelt of nodig acht, kan
hij/zij deze eenzijdig
beëindigen. Indien de hulpvrager tegen het advies in van de therapeut de
overeenkomst beëindigt, zal de hulpvrager op
verzoek van de therapeut een verklaring
ondertekenen waarin hij te kennen geeft dat hij tegen het advies van de
therapeut in het
onderzoek of de behandeling voortijdig heeft beëindigd.

De therapeut kan de overeenkomst slechts eenzijdig onder vermelding van argumenten
beëindigen, indien
redelijkerwijs niet van haar kan worden gevergd dat zij de overeenkomst
voortzet. De therapeut zal in een dergelijke
situatie hulp en adviezen blijven verlenen, totdat de
hulpvrager een overeenkomst met een andere hulpverlener heeft
kunnen sluiten.
- De cliënt gaat akkoord met de tariefstelling van €
70
voor
shiatsu behandeling,en € 80 voor
shiatsu voor swangerschap.
- Betalingswijze van de behandelingen is
online (Ideal op credit card),
op rekening of contant per
consult. Betaling geschiedt binnen 14 dagen op
rekening NL

onder
vermelding van factuurnummer.
- De cliënt verplicht zich een afspraak minimaal 24 uur van tevoren af te zeggen, anders kunnen
de kosten van de
gereserveerde tijd in rekening worden gebracht.
- Voor klachten over de behandeling kan de cliënt zich wenden tot het secretariaat van de Shiatsu
Vereniging Nederland
(

. De cliënt heeft vervolgens de keuze of
hij/zij de klacht binnen de S.V.N. wil
bespreken met een vertrouwenspersoon, of zich wil wenden
tot een klachtenfunctionaris van de SCAG, een landelijke
geschilleninstantie voor de
complementaire en alternatieve zorg. Indien een klacht niet naar wens kan worden
afgehandeld,

zal de klacht voorgelegd worden aan de geschillencommissie van de SCAG. Voor het tuchtrecht
kan de
cliënt zich wenden tot de Stichting TCZ, Tuchtrecht Complementaire Zorg (
www.tcz.nu
)
- Alle eventuele nadelige gevolgen voortvloeiende uit het achterhouden van informatie aanwezig in
het medisch dossier
bij de huisarts, zijn voor verantwoordelijkheid en rekening van de cliënt.

GENERAL TERMS AND CONDITIONS FOR TREATMENT
- The therapist is required to act in accordance with the statutory regulations. The rights and
obligations of the client and the care provider are laid down in, among other things, the Medical
Treatment Agreement Act (WGBO) and the Healthcare Quality, Complaints and Disputes Act
(Wkkgz).
- The therapist maintains a client fi le. The client has the right to inspect his or her own fi le. The fi le
is kept for a total of 20 years in accordance with the statutory retention period.
- The therapist undertakes to provide information regarding the treatment to the client in all
phases of the treatment process. The client undertakes to provide relevant information to the
therapist by means of the intake interview.
- The therapist maintains confi dentiality with regard to data provided by the client. However, with
the exception of the data for which the client has stated that he or she does not object to the use
for statistical purposes.
- The therapist may not perform certain (treatment) treatments without the client's permission.
- The therapist undertakes to correctly refer the client to a fellow therapist or a physician if her
treatment is not suitable and/or adequate.
- Termination of the treatment can take place at any time by mutual consent.
If the client no longer appreciates or deems it necessary to continue the agreement, he/she can
terminate it unilaterally. If the client terminates the agreement against the advice of the therapist,
the client will sign a statement at the request of the therapist in which he/she indicates that he/she
has terminated the examination or treatment prematurely against the advice of the therapist.
- The therapist can only terminate the agreement unilaterally stating reasons if it cannot
reasonably be expected of her to continue the agreement. In such a situation, the therapist will
continue to provide assistance and advice until the client has been able to conclude an
agreement with another care provider.
- The client agrees to the rate of €70 for shiatsu treatment, and €80 for shiatsu for pregnancy.
- Payment method for the treatments is online (Ideal on credit card), on account or cash per
consultation. Payment is made within 14 days on account NL46 ABNA 0838 6467 94, stating
invoice number.
- The client is obliged to cancel an appointment at least 24 hours in advance, otherwise the costs
of the reserved time can be charged.
- For complaints about the treatment, the client can contact the secretariat of the Shiatsu
Association Netherlands (
https://www.shiatsuvereniging.nl/)
. The client then has the choice of
whether he/she wants to discuss the complaint within the S.V.N. with a confi dential counselor, or
to contact a complaints offi cer of the SCAG, a national disputes body for complementary and
alternative care. If a complaint cannot be handled satisfactorily, the complaint will be submitted to
the disputes committee of the SCAG. For disciplinary law, the client can turn to the TCZ
Foundation, Disciplinary Law Complementary Care (

)
- Any adverse consequences resulting from withholding information present in the medical fi le
from the GP are the responsibility and expense of the client.
