Uw privacy
Voor een goede behandeling is het noodzakelijk dat ik, als uw behandelende therapeut, een
dossier aanleg. Dit is ook een wettelijke plicht opgelegd door de WGBO. Uw dossier bevat
aantekeningen over uw gezondheidstoestand en gegevens over de uitgevoerde onderzoeken en
behandelingen.
Ook worden in het dossier gegevens opgenomen die voor uw behandeling noodzakelijk zijn en die
ik, na uw expliciete toestemming, heb opgevraagd bij een andere zorgverlener, bijvoorbeeld bij de
huisarts.
Ik doe mijn best om uw privacy te waarborgen. Dit betekent onder meer dat ik:
- zorgvuldig omga met uw persoonlijke en medische gegevens;
- ervoor zorg dat onbevoegden geen toegang hebben tot uw gegevens.
- Als uw behandelende therapeut heb ik als enige toegang tot de gegevens in uw dossier. Ik heb
een wettelijke geheimhoudingsplicht (beroepsgeheim).
De gegevens uit uw dossier kunnen ook nog voor de volgende doelen gebruikt worden:
- om andere zorgverleners te informeren, bijvoorbeeld als de therapie is afgerond, of bij een
verwijzing naar een andere behandelaar. Dit gebeurt alleen met uw expliciete toestemming;
- voor het gebruik voor waarneming, tijdens mijn afwezigheid;
- voor het geanonimiseerde gebruik tijdens intercollegiale toetsing;
- een klein deel van de gegevens uit uw dossier wordt gebruikt voor de fi nanciële administratie,
zodat ik of mijn administrateur, een factuur kan opstellen.
Als ik vanwege een andere reden gebruik wil maken van uw gegevens, dan zal ik u eerst
informeren en expliciet uw toestemming vragen.
Deze gegevens in het cliëntendossier blijven 20 jaar bewaard, zoals in de wet op de
behandelovereenkomst wordt vereist.
Privacy op de zorgfactuur
Op de zorgfactuur die u ontvangt staan de gegevens die door de zorgverzekeraar gevraagd
worden, zodat u deze factuur kan declareren bij uw zorgverzekeraar.
- uw naam, adres en woonplaats
- uw geboortedatum
- de datum van de behandeling
- een korte omschrijving van de behandeling: ‘shiatsubehandeling’
- de kosten van het consul

For proper treatment, it is necessary that I, as your treating therapist, create a fi le.
This is also a legal obligation imposed by the WGBO. Your fi le contains notes about your state of
health and data about the examinations and treatments performed.
The fi le also contains data that are necessary for your treatment and that I, after your explicit
consent, have requested from another healthcare provider, for example from
your GP.
I do my best to guarantee your privacy. This means, among other things, that I:
- handle your personal and medical data with care;

- ensure that unauthorized persons do not have access to your data.
- As your treating therapist, I am the only one who has access to the data in your fi le. I have a
legal duty of confi dentiality (professional secrecy).
The data from your fi le can also be used for the following purposes:
- to inform other healthcare providers, for example when the therapy has been completed, or in
the event of a
referral to another practitioner. This only happens with your explicit consent;
- for use for observation, during my absence;
- for anonymised use during peer review;
- a small part of the data from your fi le is used for the fi nancial administration, so that I or my
administrator can draw up an invoice.
If I want to use your data for another reason, I will fi rst inform you and explicitly ask for your
permission.
This data in the client fi le will be kept for 20 years, as required by the law on the treatment
agreement.
Privacy on the healthcare invoice
The healthcare invoice that you receive contains the data requested by the health insurer, so that
you can declare this invoice with your health insurer.
- your name, address and place of residence
- your date of birth
- the date of the treatment
- a brief description of the treatment: ‘shiatsu treatment’
- the costs of the consultation
